I would be curious to know what insurance deems an acceptable reason for a breast lift (not for me - I'm too scared to go under the knife!).
My surgeon has a 12 item survey for my evaluation. Little did I know that I was 12 for 12. Insurance approved the procedure in less that a week (what??, were the pre-op photos that scary?) About 900 cc were removed; and I threw away my 38E & F bras, for a more reasonable 36B/C. Although I am a bit less busty that I had hoped for, it was the best decision I ever made. If memory serves, I believe less than 500cc is considered cosmetic. I am certain this criteria was decided by a man......
This patient didn't even come close to criteria for a breast reduction. She needed a minor lift procedure and wanted her insurace company to pay for it.
